Herdman Assessment Form

Overview

Developed by John W. Herdman, Ph.D., LADC, ICADC
The HAF is a comprehensive bio-psycho-social semi-structured interview. Based upon the client's answers to interview questions, an editable and reader-friendly narrative is generated. This narrative saves counselor's time, by avoiding having to type or dictate client history. The counselor still must develop a diagnostic impression and determine the appropriate level of care. The HAF is not a diagnostic tool or test.

Standard Sections Include:

  • Presenting Problem / Primary Complaint
  • Demographic Information
  • Medical History
  • Education, Employment and Military History
  • Alcohol and Drug History
  • Legal History
  • Family History and Family Relationships
  • Psychiatric History

Optional Sections Include:

  • SASSI-3 Reporting
  • University of Rhode Island Change Assessment (URICA)
  • ASAM Patient Placement Criteria Rating

Counselor writes the following:

  • Diagnostic Impression using the DSM-IV TR Axes
  • Summary of the Assessment
  • Recommendations
